If you have Broadcom's BCM94360CD chipset for either your Hackintosh or your Mac, you may need drivers for Windows to get WiFi and Bluetooth working. After looking on the internet for many many months now, I finally found the drivers you need.
To install, you need to open Device Manager in Windows and find the Bluetooth adapter in the list of unknown devices. Right click on it, and select the "Update Driver Software" button. A new wizard will appear and will ask you if you want to update it from the internet, or from a specific location. You want to select a specific location, unzip this file [same download below] to a folder, then select the folder in that wizard. When all that is done, you should have both WiFi and Bluetooth from that card.
